PDF or HTML

You have the choice of printing out either a PDF or HMTL form. The PDF requires the use of Adobe Reader or similar, but it will give you an exact copy of the form. With the HMTL format, the actual print out of the form you obtain depends to some extent on your Browser, however, it will down load quicker.
